I'll start it off:
Not quite in the mood of that (excellent) suggestion but I'll volunteer Polio. Almost eradicated worldwide alongside smallpox etc. Big hand for the WHO on that one.

It is amazing to think how many things will die off in our generation!
Of course, the biggest culprit is the internet. Retail stores, libraries, magazine stands, barroom arguments and, of course, listening to an album from beginning to end...these are just a few examples.
And because this is a PG-13 rated site, I won't describe in detail the biggest change of all...the thing I think our kids will miss the most. And that is a more gradual, more gentle loss of innocence. How many kids today are exposed to graphic internet porn at way too young an age? It only takes one parent forgetting to place surfing controls on one PC and...boom...nine year old Johnny becomes an expert on modern interpretations of the Kama Sutra.
