A flat white is a flat white is a flat white. It's flat. That's why it's called a flat white.
No foam.
Warmed milk from the jug, sure - but that's what the spoon is for - to stop the foam while the milk pours into the coffee.
Please, no pretty patterns in a half-centimetre thick layer of foam in my flat white. If I want to see barista skills I'll order a latte or a cappucino.

I quite agree. However, there are few cafes that have the skills and/or beans to make a good short black - and a short black is definitely what I prefer - but I gave up a while ago and just have short blacks at proven cafes.
