Customised Brass AV Wall Plates

Does anyone know where I can get some customised wall plates made up using a blank brass double wall plate? Requirements are for 2 HMDI sockets, a Radio and TV Coax and two 2 Cat5e all on one plate and a second plate that has Dolby 5.1 speaker configuration for 4mm banana connectors. Various sites have combinations but not the one's I require. I want to be able to supply the blank plates, as they are from a specific electrical range of wall plates not readily available.
